Sign-up/Change Member Information

Download the MAPS Membership Form and follow the instructions below:

1. Read the Articles of Agreement if you are a new member.
2. Complete the highlighted blocks LEGIBLY and use your full LEGAL name.
3. Submit form in one of the following methods:
   • Send digitally via email to both the MAPS Secretary and MAPS Bookkeeper.
   • Place in the Tru Sky Credit Union drop box
   • Place in the EA Bookstore drop box
   • Send to your MAPS representative, if known, who will forward to MAPS Officers
